Clinical Overview:
Experienced and Dedicated Clinical and Operational Management Team
The Legacy Healthcare Services rehabilitation programs are supported and directed by a management team of Therapists that exhibit clinical, operational, and financial expertise. The Regional Vice Presidents and the Area Rehab Managers are all licensed therapists, allowing for enhanced clinical programming and qualified direction of the facility based staff.
Area Rehabilitation Managers are responsible for the clinical, fiscal, and operational management of generally up to 10 facilities in a specific geographic area. They have strong working relationships with both the customer facility and area management teams. The Legacy Healthcare Services Area Rehabilitation Managers are also responsible for managing the Legacy Excellence Program (CQI), staff orientation and education, facility program development, and meeting any of our customer needs.
Rehabilitation Directors are typically dedicated to a single facility and are responsible for the management of the facility rehabilitation staffing, implementing facility specific clinical programs, and the communication/coordination with the Administrator and Director of Nurses. The Rehabilitation Director is also responsible for coordinating the management of service delivery, optimizing functional outcomes, meeting assessment timelines, and assuring the appropriate quality and mix of therapy services.
Program Development
The primary focus of the Legacy Healthcare Services rehabilitation team is to provide the highest quality therapy services to our rehab patients. The therapy team will provide routine screenings for residents to identify any problems or conditions which may require skilled therapy services. Legacy Healthcare Services therapists utilize our clinical program manuals and materials to assist the facility staff in developing, monitoring, and improving key patient care programs. All departments are equipped with manuals such as rehab and restorative dining, wound care, dementia, and dysphagia management. Specialized modality programs are also available with clinical interventions that have been very successful in decreasing contractures, decreasing pain and edema, improving wound healing, and decreasing incontinence episodes.
Excellence Program
The Legacy Healthcare Services Excellence Program was developed to measure the quality of our services, the efficiency of our service delivery, the compliance in state and federal practice standards/regulations, and our ability to meet the expectations of our customers. Some of the key components of the Excellence Program include a peer review process, outcomes measurement system (TAPS) review, ADR/Denial tracking and analysis, survey compliance, credentialing and licensure review, competency measurement, in-service education, risk management and infection control. These are monitored at the facility site, area, and corporate levels.
